Before we complete your NAD+ pre-qualification, please confirm you understand the next steps. This helps your clinician review, pharmacy processing, and shipping move fast and smoothly.

• Approval timing: I understand clinician review is typically completed within 6 business hours after I submit my intake (sometimes sooner).
• Order confirmation: I understand I will receive an order confirmation immediately, and I will receive a tracking number only once it ships.
• Shipping timeline: I understand orders typically ship 3–5 business days after approval and normal delays can occur (holidays, weather, carrier delays).
• If the clinician has questions: I understand a clinician may text or call me using the number I provided.
• Patient portal (best contact method): I understand the best way to contact our team is through Elation Health (similar to MyChart) for billing, dosage questions, address changes, and support.
• Portal invitation: I understand I’ll receive an invitation to Elation Health after submitting my new patient forms.
• Medical intake forms (required): I understand after purchase I’ll receive an email with my medical intake forms, and they must be completed for any prescription to be reviewed by the pharmacy.
• Check spam/junk: I understand I should check my spam/junk folder so I don’t miss intake forms, approval updates, portal access, or shipping notifications.
• Shipment notification: I understand the pharmacy will send a text or email when my order ships with my tracking number.
• Best contact info: I confirm my phone number and email are correct so I don’t miss approval, portal access, or shipping updates.
